Thank you for helping make 2023 another successful Ottawa 60+ Slo-Pitch season. We are re-introducing our annual player survey which is attached. This simple survey is in a fillable pdf format so all you have to do is enter your responses directly into the form and email the completed form back to us at ottawa60plusslopitch@gmail.com. Please note also that you will need to save your completed form first and then attach it to your email – otherwise your input will not appear.
